Who is it for?

The Brain Injury Rehabilitiation Trust (BIRT)'s outreach service is for people with an acquired brain injury (ABI) who are medically stable, want to live at home and who need transitional or ongoing support in the community.

What do we offer?

We offer a flexible outreach service with access to clinical services such as psychology, occupational therapy, speech and language therapy and a team of fully trained rehabilitation support workers.

All individuals receive personalised rehabilitation plans delivered in their own homes. These plans are designed to help people develop and practice independent living skills in a familiar environment.
