Case management
The Disabilities Trust Case Management Service operates nationwide and is lead by Alex Garden who has over 25 years' experience in social work and rehabilitation care management, and independent case management, specialising in acquired and traumatic brain injury.
The Disabilities Trust Case Management Service can:
provide assessment reports and recommendations for treatment, support and rehabilitation
provide witness testimony including single joint expert
implement treatment, support and rehabilitation plans
recruit, train and supervise support staff as required
provide risk management
monitor and review treatment, support and rehabilitation plans
provide interim reports and ongoing recommendations
provide cost of future care reports and evidence based cost of future care reports to assist in costing the claim.
