The Disabilities Trust, including the Brain Injury Rehabilitation Trust, has a wide range of services in locations across the UK, to work with people with an acquired brain injury and physical disabilities to help them live as independently as possible. For over 40 years, our high-quality services across the UK have supported people to move forward with their lives. These include: brain injury assessment and rehabilitation centres, hospitals, care homes, supported living accommodation, care in people’s homes and a school.

We accept referrals from professionals working with individuals who may benefit from our care and rehabilitation services.

The referral process

1) Referrer sends information so we can understand the needs of the individual (by email, post or online).

2) We contact the referrer to arrange a face-to-face pre-admission assessment, held where the individual is currently living.

3) We send the referrer a report of our findings, with recommendations for care and support, including costs.

4) Once funding confirmation is received a ‘move in' date can be agreed.

We will maintain contact with you throughout the referral process.
