The Disabilities Trust, including the Brain Injury Rehabilitation Trust, has a wide range of services in locations across the UK, including hospitals, neurorehabilitation centres with transitional living facilities, centres providing continuing rehabilitation and specialist care for people with longer term needs and supported living and ‘at home’ care and support.

We accept referrals from professionals working with individuals who may benefit from our care and rehabilitation services.

The referral process

1) Referrer sends information so we can understand the needs of the individual (by email, post or online).

2) We contact the referrer to arrange a face-to-face pre-admission assessment, held where the individual is currently living.

3) We send the referrer a report of our findings, with recommendations for care and support, including costs.

4) Once funding confirmation is received a ‘move in' date can be agreed.

We will maintain contact with you throughout the referral process. If you use the online portal you will be able to see the status of your referral, and add/receive comments with our team as this progresses.
